WBMA's Insurance Carrier

WBMA's insurance carrier is Premera Blue Cross. WBMA acts as the administrator for the plan; handling all billing, additions, deletions, changes and assisting with questions regarding coverage.

Employees are eligible for the plan if they work 20 or more hours per week. There is a 90 day waiting period prior to enrolling on the plan.

Children are eligible for the plan until the age of 25 as long as they meet the IRS criteria of a dependent..

The following Medical, Vision and Dental benefit information is for the August 1, 2007 to July 31, 2008 plan year.

We receive many questions about COBRA regulations. If your firm employees for 20 employees over half of the year, then the firm is required to offer COBRA benefits to their employees. Click here for a copy of the COBRA regulations and sample notices.

Medical Coverage

WBMA has three $25 office visit co-pay/PPO (preferred providers) deductible plans available to the membership.

Included in the medical premium is Vision coverage through VSP (Vision Service Plan) and prescription drugs which are $15 for generic, $25 for preferred name brand and $45 for non-preferred name brand.

We have three deductibles to choose from which are for a calendar year:

Vision Coverage

Vision coverage is included in your medical premium through Vision Service Plan. There is a $10 exam co-payment and a $25 hardware co-payment and if you choose a preferred provider, the plan covers one examination and one pair of glasses every 24 months.

Dental Coverage

The dental plan has a $50 per person deductible with a $1,500 yearly maximum. This plan covers 80% of most routine services and 50% on most major services.

Life Insurance

WBMA has a life insurance plan through States West Life with a maximum of $20,000. If you wish to purchase additional life insurance, we have voluntary term life insurance available to our members through CIGNA Group Insurance.

Accidental Death and Dismemberment

WBMA offers accidental death and dismemberment benefits in the amount equal to your life insurance through States West Life. Additional accidental death and dismemberment benefits are available through The Hartford Group.
